The applicant's name was Shavvin Gajrhena Neyakat (changed to protect his/her identity) as seen on the form A of the application.On his/her M.B.B.S. and M.D. degree certificates, it was Shavvin Neyakat Gajrhena. On the undertaking he/she had given on a stamp paper of Rs. 100/- it was Gajrhena Shavvin Neyakat. For ease of understanding, I shall put it in a tabular form.
